Act 3 Cruel - Jiquani's Machinarium does not spawn first small soul core

Could not find the small soul core quest item for the first door in Jiquani's Machinarium zone in Act 3 cruel. Noted that Delwyn was present but also no wisp in a section of the map.

Had to open a new instance of the map to find quest item.
