Weapon swapping bugs out and disables attack skill

I have a weapon swapping character built around Ice Strike and Rake. It will work for most of a zone, and then after a while Rake will stop working entirely. It did work before.

It does not say that I lack requirements to use the skill in the skill menu while it is bugged. All my stats are correct to use all gear, and it was working previously. It just stops working on its own after a time in the zone, or after swapping enough times.

If exit the game and restart, the skill (Rake) works in conjunction with Ice Strike once again, but again, after 1 zone or so Rake disables itself after weapon swapping.

Note: I have the attack skills swap weapons automatically. I am not using the weapon swap key to change weapons. Also, using the weapon swap key after it is bugged does not fix the bugged skill (Rake).
