Chest loot & UI glitch which removes my A button on console until I relog.

I just recently rolled a strongbox with Contains an Additional Unique Item, completed the strongbox and received nothing but some augs and some blues.

Secondly I would say around 1 in 10 chests I roll result in my losing the ability to highlight items with a visible button to allow loot or interaction with breakables, shrines, etc. I have to relog in order to fix this issue.
