Need some help (ssf)

pob above is my toon, i am currently stuck at 89, it was super smooth sailing until then and all of a sudden everything kills me, i had to go from yellow t15 down to blue t15 just to dodge certain mods, especialy extra damage or monsters deal increased damage, was realy tanky before the patch so i dont know what changed hence why i ask here for help

damage surprisingly is actually pretty fine as it is right now, need to fix my survivability, also i cant reliable kill ultimatum for 3 pices, got one and used it on the door, didnt know i need 3

anyway, if anyone can help improve my build, i would realy appreciate it, its a homebrew build and i actually like it
