Unable to attack not stunned

Anyone else ran into this where you are spamming atacks in say a breach, you are NOT stunned and your keeping mana up yet some reason you all of a sudden cannot attack ??? like your smashing the button but the game justs eems to skip your attacks for a period or on and off ??
