Instant death

I was wondering if any experience this. Me and my friend are playing i am witch inferolist we both die when are full health and when I soloing we didn't even get hit. yeah my res around 70ish. We understand that part. The thing was we where no near that. I was doing a teir 13 or 12 handle it find but when I tier 3 vaal just one shoot out no where. I even use the bodies to make minion so they don't explode. We no nears pools we look it up to apparently there bin several case that my friend found we check my passive and Items.yeah any information helps we where doing the trials out no where it's says we died and still had full health and energy shield. There was a bug that I couldn't even grab things I teleport, inside, what are the pools of the devour? And each, for some reason, uh, my friend just hit, he dies automatically but when play like all damage goes towards him or me.there bin bugs where i got suck in circles where devour spawn. I couldn't I couldn't grab any loot at interface is all over the place.

Edit also identifying scrolls don't drop up at all.When we are you doing the maps
