Vsync causing constant frame spikes at regular intervals

Im getting these frame spikes almost the whole time while playing. Some times they stop but will start again soon after. I have vsync set to adaptive and when its off the frame rate is fine but I still have screen tearing even when the cap is set to my monitors refresh rate. If I uncap the fps I dont have tearing but videos on my second monitor stop playing. I've also tried locked but it still happens. I've tried all 3 rendering modes and they all have the same problem. I have a RTX 4060 and i5-13400F if that helps.
