Memory tears not spawning after the quest ones

I haven't seen any memory tear (to start the memory chain) after completing the 3 quest ones. The quest ones appeared immediately, but not a single one after them.

I was thinking I'm going insane since everyone was finding them without major issues, but then I found similar bug reports. In particular someone checked the logs so I did the same thing.

Here is the log of the last Eagon chat:

After that I've been mapping for 5 days, there are 157 logs of "Generating level 8X area "MapWorlds...." after the quest. Not a single Eagon chat.

Something is definitely off. Or am I just blind and not paying enough attention?
