Omen of Catalysing Exaltation bugged or just unlucky?

Like the title says, it feels to me that this omen is bugged and does not apply when used in combination with other exalt omens. Would love to hear if other people have a similar experience or counter examples of it behaving as expected with multiple omens.

I have used it 6 times so far, with 3 of those being just the omen by itself and 3 other times with various combinations of prefix/suffix/double omens. All slams where with 20% quality and Greater Exalts.
When I used it by itself it always hit a mod with the respective tag (outcomes: flat lightning with lightning cata on ring; life with life cata on ring and dexterity with attrib cata on ring), while it never did when used in combination with another omen (try 1: life cata with double exalt and prefix omen on amulet, hit %es and spell dam; try 2: attrib cata with suffix omen on ring, hit all res; try 3: fire cata with suffix omen on amulet, hit crit dam).

While this sample size is obviously too small to conclude it's outright bugged, my experience with using it definitely makes me suspicious.

I also suspect there may be a bug here, although as you say the sample sizes are limited so as to bias us.

In my case I have an amulet with spirit/flat ES/%ES on prefixes and empty suffixes. I've sunk a ton of currency into catalysing exalts, both greater and perfect, and none of them have hit the applied quality tag.
