Cataclysm's wake

The Quest Cataclysm's wake is severely bugged. After the first completion of a Nexus I did not get anything. After finding and clearing the second one, I got the quest for a "complete a Tier 3+ Corrupted Nexus" now, after completing it with a Tier 3 waystone, nothing happened again. No progress, nothing.
