Forge Hammer serious throw bug

There are some instances where when you throw the forge hammer, it doesn't come back.

I see it happening in some maps with big off screem areas, like in that arena with the Colossus boss.

When it happens, you can't attack again with the skill and need to go to your hideout and back to the map again so it re-enable.

This can be quite serious when you are in a boss or a big group of monsters.
