Item disappearing from inventory

After completing a map, I throw another map in the map device and run it. I identify my items with Helena and open my stash. When the portal opens to the new map an item from my inventory vanishes. In this case it was 1 chaos orb.

I'd like to also note when I first open my stash tab I have 1272 chaos orbs. After that 1 chaos orb from my inventory vanishes, I check back to my currency stash tab and it changed to 1269 chaos orbs.
