Queens Hunger + Leash of Obit killing me?

Queens Hunger + Leash of Obl + Catarina Ascendency. I have 16+ second uptime on my offerings, which should cause them to never fall off and i should never die. But i randomly do, and there appears to be no reason? i cannot figure out why i kep dying. its not a mana issue, and im not cursed when its happening.

this bricked my entire build idea, and at level 90! plz help :(

Does this happen in hideout too or just in-maps?

Are you running maps with reduced buff duration?

This happens even in low level campaign areas, and in some cases in hideout too. Definitely happens more often in combat but again there should be no mana issues which was my first guess.

I ran low level white maps, no mods mostly.

What intervals do you die?

I thought it would just straight out kill the player on offering refresh.

It's pretty specific which is why I thought it would work "does of it falls off" and that's exactly how it works luckily! Since they are always "on" you don't die
Except, completely randomly and with no discernable pattern. there's no Interval I can see. it happens completely at random, which I'm sure isn't actually the case.
