Shadowed Blood of Assassin still gets hurt by Monsters Poison on Hit???

If I have the ascendancy shouldnt this damage be completely negated? Yes still affected by poison but damage shouldnt be happening? I am also on CI so I SHOULD be immune to chaos damage, but I am not. I almost died to a couple of normal mobs.

P.S. Almost died taking that screenshot, was only able to clear map due to ES Leech and ES on hit Watcher's eye.

Upon further testing and video recording, I am using a Bladefall of Trarthus. Using it normally is sustainable, but the moment I get poison, I get damage over time. How is this interaction working for anyone more knowledgeable than me?
