Omen of Amelioration bug

I have been frustrated with this omen for a while now. Hoping it would get fixed. Yet it still remains. When it does work, it will work once and when I put another one on if I play the same map or within the same content it will not prox again. Or it will just not work. This issue has drove me to quit play mid season as I am striving to hit level 100 and my progress will be hindered so much from a death that it is day worth of play down the drain. Please fix this Issue ASAP.

That Omen says 1 omen per Instance (map) in the description. It's not a bug. Also it doesn't work with the omen of Resurgence as it's also 1 omen per instance- they are either/or not both.
And turn Amelioratiin off an any areas like Twisted Domain / breach boss, even though you dont lose XP in there it will still waste them if you die.
