Game crash, cannot progress at all! PLEASE HELP!

Every time I finish first phase of the act four final boss my game crashes and resets the boss. Anytime I also complete any quest my game crashes and resets the instance. I cannot progress at all until this is fixed. I was really hoping to get to maps today please help. I’m on Xbox.

In trials of chaos i cant use soulcores, Switches or talk to trialmaster, instant dc.

Same at jinquari, Grab soulcore, put in, dc.

Cant play at all.
