Skill Book disappeared

I just killed the Blind Beast in Act4, the skill book for 2 passive points dropped, then disappeared in my inventor (no I did not click it). I tied to load a new instance, connect to a different server and speak to all the NPCs in town. The quest (symbol with the +) is greyed out on the map. My skill book is gone and so are my 2 passive skill points I should get. (I double checked, I have all the skill points until this point so far, and did not receive the ones from this quest).

What am I supposed to do to get this back?

I got the skill through another quest, so it seems they changed where the passive skill rewards are acquired from.
Still, that the book dropped for me in the first place is a bug!

I encountered the exact same bug. I saw the book drop and picked it up but it wasn't in my inventory and didn't grant me the 2 points.

Same issue here, 22 points collected only, blind beasts book disappeared, David Blane give it to me back ;(
