Passive Skill Point Book Not Dropping After Blind Beast Kill – Game Crash (Act 4)

After killing the Blind Beast in Act 4, my game crashed immediately. When I logged back in, the Book of Skill that grants the passive skill point did not drop. I have since reset the instance and killed the boss again but the book still does not drop. Looks like it permanently locked the reward after the crash.
