Happened to me too, on Flotsam map. Telzaro spawned from the league mechanic in the map, and now appears stuck underground and looks like a magma puddle. I can't hit them, but they can hit me if i stand on a specific spot (see screenshot). It happened while i was hitting them with tempest flurry, which pushes opponents around. It's hard to tell, but the bug might have happened as they got pushed against a wall / cliff.

Happened a second time in a row, this time in Trenches. This boss seems very vulnerable to knockback effects, and i'm 99% sure the bug happened because i pushed him thought a cliff using Tempest Flurry. My guess would be that the skill's very fast hit rate and the boss's weakness to knockback don't play well.

The boss still emits some particle effects through the ground, but they dont show on the screenshot so i circled his approximate location.
