Item disappeared

Hello,

I was doing event Vaal Ruins. Finished the layout and started to progress, I had put my talisman 2 times in a rare equipment slot to add another stat, got my talisman back from the second bench then I was moving to the next room and game disconnected from the server and returned me to the login screen. I logged in and I didn't have the primal talisman weapon anymore. It was a very good item with %100+ physical dmg , atk speed and +3 melee skills.

Vaal Ruins progress got reset. 2 previously active room has vanished and I couldn't enter the Ruins after reconnecting.

Can you help me out ?
