Missing Passive Points (with proof!)

I have compiled an imgur album showing that I have checked absolutely every quest reward and boss in the game:

I'm missing two Weapon Set Passive Points (I have 22, not 24), I have killed all the bosses that grant passives again (that were possible to kill again) and checked all NPCs that award passive points from quests (and every other NPC to be safe) and I absolutely cannot find these missing passives.

I have the same issue - I posted yesterday asking for help. No response yet.

Oh yeah here's an imgur link to a screenshot showing I only have 22 Weapon Set Passives allocated, and that my max is showing as 22 (not 24), so it's not just a visual bug or anything.

I have the same issue. I completed 2 main quests which supposed to give 2 passive skill points each, but didn't receive them.

Holy fuck I'm genuinely so stupid I thought 'Wolvenhold' was the area with the final Interlude boss in Ogham Interlude, and that the final boss gave the skill points. This is not true, Wolvenhold is a side area that wasn't showing on my map so I never noticed the +2 Passives marker. Just got my passives.

Which quests are you talking about? There were some changes to where you receive passive points this patch. Probably a good idea to double check your progress against this guide:
