Dying with 3 portals and losing the entire map

My character died on the map, I got a gigantic loading screen that threw me back to the login screen, when I logged in, the portals were gone and my map showed as if I had failed and my tablets had one less use. Not to mention several other times the game logged me out and when I returned the same problem happened, even without having died. It's starting to become impossible.
