Act 4 Quest: Confront Kanu and the First Herald - can't progress

Killed the boss as he killed me and clicked resurrect. The door doesn't open and the NPCs don't show up to open the door and walk through the site.

Resetted the instance, the boss says a line when I reappear and flies away, but the door stays shut.
