(Xbox) After patch 0.4.0c, disappearing textures continue and more

The problem with disappearing textures is still just as severe. Additionally, there's a critical system and audio crash bug happening, where I can't even open the xbox menu to close the game.

The disappearing textures is happening in co-op or single-player, but I've only experienced the audio/system lockup when I was using my headset in co-op so far.
