Simultaneously dying to Benedictus while you kill him will brick the area and force a new instance

I died to Benedictus at the same time I killed him with poison, which caused his fight to bug out and none of the scripted events will occur now, even if I respawn at checkpoint.

This occurred to me this morning. I died and then the boss died. After re-spawning the door is not open nor can I fight him again.

After resetting the instance I was able to fight the boss again and move forward in the campaign.

Yeah, you unfortunately need to go to the waypoint and use the Instance Manager (ctrl click on the waypoint map) to create a new instance. You'll need to re-do the map and boss fight in the new instance to get past this.
