Did not get 4th ascendancy

I completed a full trial of chaos but did not receive my fourth ascendancy, it was ether level 78 or 79. I am including my bug report number proving i completed it. The alter says to interact to gain my points, but I am not getting them.

Wait so i have to complete the trial three times to get the pieces or buy them in order to get the points through the chaos trial?
