Atziri Medallion Bug

Not sure what the trigger is of this issue. But Atziri dies and goes through entire death animation. Her medallion drops on the floor, it says that you cannot pick it up because "medallion inventory is full" i had 0/6 medallion slots filled, despite this it still would not let me pick it up. After leaving the cradle, the medallion never respawns anywhere in the regular vaal ruins after the fact and appears to be stuck in the cradle I guess? You cant travel back through the portal as Doryani destroys it so this forces you to reset the entire temple. This has happened to me twice now.
