Random Teleporting / Desync

Im not really sure which to call it. I will be clearing a map and all of a sudden im teleported into another random pack... sometimes its a short distance, other times it looks like 2 full screens. Does anyone have any info on this? Thanks

10 minuites after making this i teleported into a room that had closed doors....
