Sinking Spire Tower Map generated with no door to the 2nd floor

Entered a sinking spire tower with a 4 mod rare TXV map with Abyss and Grand Project tablets. There is no active door to the 2nd floor of the tower. I can see the opening that should have a door in the southwest corner but there is no checkpoint or active door pathway.

There is no door in either of the 3 points of the triangle or around the central area.
