Omen of Resurgence Not Working

Have turn on only one Omen of Resurgence in my inventory but it's not even work just die like by one tap, and omen still stay turn on in inventory after resurect, Wtf!? Please fix this bug

I've run into the same thing. Greater Exaltation and Omen of the Ancients don't seem to be processing either.
Arguing on the Internet: What's the point when you can't punch them in the face when they really piss you off?

Not a bug, if you are hit and went from 100% to 0% life then you were never in the zone of "low life" for it to be activated. Say you had 100 life and that one hit did 65% damage to your life then the omen would activate since your life reached the criteria for the omen to be used but 100% damage to your life the it's instant death.
