problem with passive skill points

I just now noticed the last two are missing. I've been running through the maps for a while now. I checked all the bosses, they're all dead, and all the quests are complete. I talked to all the NPCs in all the acts, but someone doesn't want to give them to me.

Where could I have lost them?
