my medallion can not be collected.

The Fate of Vaal mechanic is incorrectly indicating that I am owner of the map,my medallion can not be collected.This bug occurs in co-op mode, yet it persists even when I switch to single-player.

When I enter the Grand Phylactory room, the boss mechanism doesn’t appear, so I can’t complete the objective.
