Vaal ruins

I am constantly experiencing doors that show opened and closed when they are the opposite. Also regarding the ruins I dont understand why things all hang out at the only door entrance so I can't make my way into the room to complete it. Also I got stuck in a room because a npc ran out of the room and I was unable to kill him since I was locked into the room so I just had to accept my fate and die
