Temple Surgeon Upgrades

Why is the operating table not providing upgrades to what you already have, but it always gives you other options? So, let's say I got on left arm attack speed 7, but I want to make it 10... How? It won't provide such an option. I have to pick some bs like block chance or curse magnitude, then pray next surgeon will give me attack speed and also prey it's going to be more than what it was before? So I have to build several T3 surgeons in a row and keep locking them, because... WHY!?
