Omen of amelioration not functioning

Omen of amelioration not triggering during death. Appropriate red indicator is on indicating active.

First, is this for PoE2? If it is, it goes in the "Early Access" section of the forum, not here.

So from here I'll assume it's for PoE1:

Is it a death to a pinnacle boss?

Pinnacle boss zones have the mods "players do not gain experience" and "players do not lose experience on death", so they shouldn't use up an omen of amelioration anymore.

If not, the only other explanation I can think of is the limit of one omen use per zone, so if you also had another omen in your inventory (such as an Omen of Death Dancing) that triggered first, that would stop the Omen of Amelioration from working.
