I can confirm. It's the chat messages, even when hidden or when my own character is talking.
Should not be an issue on 14900k + 4090

Same issue. Entering a chat that is not heavily occupied fixes this. Its very easily replicable and GGG can easily test this themselves.

Can confirm switching chat solved the CPU spikes. I was in Trade 1 without knowing. 7800X3D/RTX 3080.

Also having this problem, but can't change chat channels because I'm not level 25, yet :( Sucks having to restart the game so often.

I would highly advise against doing the 100% CPU usage. I tried the script for the power plan and it put my CPU at 85C constant temperature while playing which is very bad for the sillicone components on the long term.

On the other hand, I tried the chat switch solution and it did seem to improve so thanks for that suggestion!

My Specs, GTX 2080 Super / i9 14900k using a 4k Monitor and mind you I can play most games on max settings and average over 120FPS. When I first downloaded POE2, I had no issues with stuttering or lag. But once a recent patch happened, My Game stutters constantly and moments my Game will freeze for about 1-3 seconds then hyper speed fast forward back to the present. I for 1 will also not recommend setting your CPU to 100% temp, no reason to kill your PC to solve a problem that the game devs should be hotfixing. I also turned off chat via hiding them and the Stuttering has calm down alot (not perfect) but the game is playable. IDK what GGG did on that patch but whatever happened they broke something.

Another fix if you still want to be in global/trade 1:

Rename/delete the client.txt in "steamapps\common\Path of Exile 2\logs"

Got rid of my 100ms+ cpu spikes, will most likely do this every week now :D
