Balbala doesn't drop quest item

So I was able to get it to drop the first time, then I went through the trial and beat the first boss. I then got smoked by a terrible roll (40% reduced move speed or something) walked into the next room and immediately got gatted down by a couple dozen ranged mobs. I then got a message about having additional tries, and clicked the button but it started me at the beginning with the Barya missing and the door closed. I have then gone back and tried to kill her again for the Barya but one hasn't dropped. I saw a comment about it possibly being related to culling strike, but even killing her without it she won't drop it again. At this point the quest is just stuck, and keeps telling me to search the Traitor's Passage, and there's no way to get the item.
