Witch Animation/VFX hard crash to desktop - multiplayer & singleplayer

I am getting a hard crash upon casting certain spells. The spells that have crashed me include:
- Frost Bomb
- Ice Nova
- Comet
- Frost Wall
- Fireball

The crash happens somewhat inconsistently but frequent enough to hit it within ~5min of killing packs. Essentially I'll cast one of the listed skills and the game will instantly freeze and crash to desktop. Sometimes this happens when enemies are frozen or shatter, sometimes it happens if I cast too many skills on the floor of an empty room.

Playing a Blood witch, if that could be relevant.

This crash affects not only me, but anyone within range to have the cast loaded. Multiplayer partners that are far enough away are unaffected, but anyone sharing the same screen/on the edge of the screen also instantly hard crash to desktop.

I have tried:
- Vulkan, Dx12 and Dx11
- medium & lower texture quality via ini tweak as shown in https://www.pathofexile.com/forum/view-thread/73322
- dynamic res on/off
- GI & shadows low/high
- vsync on/off
- settings all graphics options to lowest possible quality
- removing afiliation with cpu core 0 and 1 as some posts recommended
- Verify integrity of game files via steam
- shader cache deletion
- full reinstall
- pc restarts

I am running win10 10.0 22H2 Build 19045.5247 on a Ryzen 9 7950X3D, 64GB DDR5 RAM, GeForce RTX 4070Ti Super

I've also got the memory dump here, obviously I can't debug this but would be happy to send it if you tell me how.
It's just for an exception 0xc0000005 saying "The thread tried to read from or write to a virtual address for which it does not have the appropriate access."


Edit:
This was originally just a frost spell bug, but since swapping to a fire blood witch I'm having the same problem, though it takes longer for the bug to happen.
One thing I will note, if I play my flicker/lightning monk I don't have this issue despite also spawning enormous amounts of vfx with charged staff, bell, lightning strikes & elemental expression

Now the monk is also crashing. Though it look nearly 40 minutes
.dmp had a different error though: 0xe06d7363
No exception description
I found the error log though, looks like texture resources are failing to be created:

Seems the two crashes with the monk and the witch are not directly the same, but will keep all this here anyway.
