Forge Map Bug(s)

After clearing the Forge map(T5), I came across a guarded box on a bridge path. When I clicked, it generated the fog but no guarded mob packs. I assume is spawned them under the bridge. It would not complete.

On the same map, I also noticed there was 1 enemy left and it was a boss that was inaccessible (he was in the extreme south, off of the map) and I could not complete the map.

Not sure if these are separate issues or related.
