Chaos Trial - Not Opening - "Operation could not be completed"

In the same situation in chaos trial.
Died to the boss , but also killed it, it bled out :)

This Happened to me as well, But... I found a work around.

I took my "Chimeral Inscribed Ultimatum" out of the alter. Then walked back out of the area to the Chimeral Wetlands. After I loaded the wetlands map in, I used Ctrl + LClick to bring up the instance menu, and started a new instance.

After that I was able to place the Ultimatum into the alter and enter again.

Weird bug, but at least there is a workaround.

Thanks for this suggestion, it worked for me. I failed the Trial and grabbed my Inscribed Ultimatum from the Temple Offering statue. I did the same, exited to Chimeral Wetlands and Ctrl + Left Clicked on the words / name of the zone "The Temple of Chaos" and this opens a menu to create a new instance. New to PoE in general and didn't know this kind of menu existed, so thanks for the tip.

I am also having this issue. This has really killed my interest in the game. Having to go outside the game to trade for the gear I need to continue is a real buzz kill and it makes me sad that what you need can not be found in game. This is the same thing that killed D3 when the economy went bad and everyone stopped playing
