Happened for me as well, but i can add, i had a stairs, that vanished once i killed a monolith boss (a boss spawned by clicking 3 monoliths).

Having same issues. Corrupted Nexus and no way to get to the final boss to advance. SMH

I also had this bug in PoE2 0.4.0c
I had abysses and a shrine in my map that I had completed if that matters (for the rest of the map modifiers, see image:  |
