I have the same issue, do we need to wait some time till the instance somehow resets?

I don't know if they fixed it but I did pull a bunch of cosmetics off my main to my alt, and one of them, the Twitch drop head accessory was buggy and wouldn't equip. After swapping the cosmetics I could log back on my main, maybe it's a solution?
