Just saw this in another thread, CPU spikes seem to come at the same time as chat messages in global or trade chat for me.
I just walked around my hideout and saw a spike as each message came through.
Hiding/filtering the chat didn't help.
Leaving trade 1 helped significantly.
What helped the most though was deleting/renaming client.txt at steamapps\common\Path of Exile 2\logs. It appears this file is updated with each chat message and for some reason this causes a CPU spike when the file gets larger.
Maybe delete/move/rename this file each time before you play, or close the game, rename/delete, then open the game anytime you see an issue, and see if it helps?
It helped me at least.

It works! My log file was 17megs and my game was unplayable but now after 6 maps its only 40kb and the game is smooth as silk. Thanks so much for posting this!

Fixed, well, in my case... Reset the internet router, connected to 2.5hz frequency, not the 5hz (5g), maybe for some of you folks your internet provider offers to types of speed, just like mine does. Take care!

I think it worked for me as well, i also installed newest drivers for nvidia studio as as someone mentioned before that it help also.

Yeah, it's annoying when trying to dodge something only to end up rolling into a group of mobs like 20 meters away in the blink of 1 second when the lag stops.... Fix this issue, it is game breaking

That's a weird log file ngl.. storing every single chat message since 7th December lmao. My file was 30MB.

Tried the suggested fix to removing or renaming the client.txt file, but that hasn't resolved the issue for me unfortunately.
I've noticed in game when things are stable I'm getting a MS of 8-10 with a FPS around 100.
Then I'll get a spike of MS of 13-15 followed by a FPS drop to 50.
It then recovers, rinse and repeat.
Happens every 4-8 seconds.
