For those having network spikes every few seconds, the only way I've found to mitigate this is to log in, see if it is happening, and then if it is, quit the game for ten minutes. Try, try again.
Seems to have about a 50% chance of being bad but quitting the game for a bit seems to reset things somehow. Highly annoying but better than stomaching it for hours on end.

Post made 7dec 2024 has 129 pages and still going. You guys still hope for a fix? I know for a fact that they won't do anything about it because this issue was present in poe1 too for at least 3-4 years (as long as I played) and GGG never even acknowledged it.

First Fix
GO to C:\Users\YOURUSERNAME\Documents\My Games\Path of Exile 2
Poe2_production_Config.ini
Find [NETWORKING]
Networking_type=auto change to Networking_type=predictive -> Save it
Restart the game
Second Fix
Open cmd.exe
Type ipconfig /flushdns
Restart the game

GGG use the same servers for PoE 1 & 2 , regardless of the players load the results have always been shit :
Tested at Christmas under heavy load/stress ( 400K players )
Tested right now = lower player count ever ( 100K )
To anyone wondering why is it that bad : do you seriously think the CPU stuttering caused by bad code that cripple Consoles & multi-threaded PCs doesn't cripple multi-threaded Server's CPUs as well ? It totally does .

I also figured the monk is cause the game to lagggggggg.
I have to 90+ chars my necro has almost no lag or none and when i swap to monk the stutters already start in the hideout.

I got a high end pc, and it started like thunder from a clear sky.
RTX 4080, Ryzen 7900X, 64 GB ram (6000mhz)
Still the game just suddenly became unplayable.
Hope it will be gone tomorrow.

What fixed it for me:
I had the problem that I always had to play on DirectX11, because with DirectX12 and Vulcan I only could get a fraction of the FPS to what i could get with DirectX11 (on PoE1 same problem).
Herald of Ice explosions and some other effects often result in gameplay with 1-10 FPS, which made content like Breach pretty much unplayable.
I literally tried everything that was described in other posts on Reddit and forums to fix my problems but nothing really helped.
I then noticed with CPU-Z that my graphics card only runs with PCI 3.0 x2 instead of PCI 3.0 x16, which I then was able to confirm with GPU-Z. So my graphics card was only using 2 lanes instead of 16 the entire time, which of course resulted in a much smaller max bandwidth. After a lot of research and trial and error, I was able to fix the problem and since then I can finally use DirectX12 and the Vulcan Shader without any problems. The lags from Herald of Ice explosions are completely gone too since i swtiched to Vulcan after the fix.
There are various causes for this problem: The graphics card could be plugged into the wrong slot, M2 SSDs may take away lanes from the graphics card, The BIOS may need a CMOS reset, even defects in the graphics card or one the motherboard are possible causes etc.
If you are affected by the same problem, you should research yourself how to best fix your specific cause for this problem. In my case, a defect in the top PCI-slot caused the problem, so i then switched to the PCI-slot below as the graphics card slot.
specs:
CPU: i7 8700k
GPU: RTX 2700super
32GB RAM@3000
Motherboard: Asus STRIX Z390e
Hope this helps some people!

Having the same issues and they seem to be growing worse.
Game ran smoothly the first couple weeks and then started getting lag stutters that coincide precisely with network lag on the in game graph.
CPU and GPU lag and virtually nonexistant but every time the network lag spikes the game stutters or as has been the case this last week it grinds to a halt every 20-30 seconds for anything from 2-30 seconds.
I installed pingplotter and have been tracking the washington DC server alongside google and interestingly everything is fine and smooth even with both twitch and youtube streaming at high quality. The instant I fire up the game the network starts spitting packet loss. Not just on the POE2 server but on google as well which tells me the game is inducing some terrible network performance. Not a bandwidth issue either as the game never asks my network for more than about 0.6 Mbps at any one time. No matter how I configure things and I have tried every solution listed here and elsewhere on the internet nothing relieves the issue. Last night I had almost 6 hours of smooth lag free gaming from 10pm to 4am. Today its back to unplayable.

I have had no issues up until this week. Now all the sudden every other instance is spiking up to 600+ ms when everything else in my network is fine... This game has literally become unplayable.
