I've also seen the ability to pick up items just completely disappear. No active UI element showing the button to pick up the item, all other keybinds work, just cant pick up loot. Fixable by, you guessed it, exiting to main menu and returning.
This happens to me a lot. Almost always because my guy is targetting something. And even if there's no mob around, there are things he still considers "hostile" like remaining enemy bone walls or the wheel that the guy carrying it above his head drops. Only solution is shoot and destroy them, to free up targetting.
This btw makes looting in combat almost impossible. Just wating for that divine drop followed by a rip and no looty.
Yeah I leave the interact button without a skill. If there's anything hostile on the screen, it will always prioritize the skill over the interact.
Aside from that, I sometimes cannot pick loot up because even though loot tooltip is there, the actual loot is out of reach. Happens a lot when a lot of item names are pushing each other around.

Yes leaving the interact button free of other binds is definitely the play. I had it set to portal for awhile because even if you accidently activate it you can roll-cancel the portal animation but eventually I just moved portal to a d-pad button instead and haven't looked back.
Still, having the interact button not even show up on loot at all is infuriating and makes you have to burn a portal exiting to menu to either reposition loot markers or get the interact tooltip to appear again because it broke for no reason.
Unrelated but on the subject of binds, roll on LT feels really really good. Default roll bind or any face button binds feel weird without using a paddle. Being able to roll while having both thumbs on sticks makes ranged classes feel almost superior to using KBM, mobility wise.

I believe it's that button by default since it's like an industry standard by now heheh.
