Me too... Does this only drop the first playthrough since it unlocks the forge?

Same here. Mannchild might be onto something wrt subsequent characters after your first. I could have sworn I completed this on other alts this league though.

Just completed the quest on a third character, no hammer dropped. Im going to assume this only drops the first time completed each league. Would still be nice if GGG could confirm this.

I cant get him to dropped either. I've fought him like 15 times. Trying all different kinda ways to get drop. It's annoying.

Same here - and Mektul is not crossed out on the map after killing him 10 times I think!
