I can't design well so I'll just describe it.
I call it... War of the Shirtlas! The worlds of the atlas, similar to our own world, exist in a sprawling void. While ours consists of planets and suns, the Path of Exile universe has two entities that are very much different. The Shaper and The Elder. The idea is play off the idea of these beings existing in the endless void. Similar to the chaos orb shirt in how the faces hold shadows that trail off into the black of the shirt, this ideas takes the distinct faces of The Shaper and Elder and does something similar. Shaper, for instance, would have his starry eyes and perhaps parts of his nose and beard seemingly coming out of the "void", not showing a whole face but creating a ominous, and recognizable depiction. I don't want to use the world minimalist because I think it doesn't exactly fit, but the idea is making it seem like his face has only just begun coming out of the darkness. Elder is harder because the whole "teeth and tongue" thing isn't as distinct as Shaper's eyes. There would be multiple ways to combat this. Either have some of his cloak and/or hood surrounding his gaping maw, or combine the characters into one piece and surround the above description of Shaper with the slight appearance of teeth coming out around him. Honestly this is an old, and i've been waiting for ages for an official t-shirt of this caliber.
