PoE 2 - Inscribed Ultimatum of the Chimera Disappeared

Had this item drop in act 3 - saw the green item, picked it up and got the quest for the trial of chaos.

Can be completed by joining someone else with the quest item and completing the Trial with them as leader.

I basically quit and was rerolling until I thought to try grouping with someone else that had it - fortunately I was able to carry them through it.

Yep same here aswell, have no item. but i have the quest "place the inscribed ultimatum in the temple offering".
i cannot drop the ultimatum again by killing xycuclan again. hardstuck :< |

quest item dropped picked it up nothing in my inventory can't do trial.
