It's weird that GGG has not posted anything about his on "Known Expedition issues" after several posts about it in the bug forum thread. It is definitely known... My theory is that if you use a portal (or die) AFTER you picked up the text but BEFORE you spoken to Dannigan the quest gets into this bug loop.
