POE2:Treasures of Utzaal, Act 3 Molten Vault, Quest item not dropping

Also have this issue. Maybe it's based on skill level or another factor if it drops or not. If it's bugged hopefully they fix.

I have the same problem, with a chaos minion witch. That thing what they posted is almost a year old. Realy dissappointed, cant be that hard to fix. Just put someone on that ticket please

Did Mektul talk to you before combat? IIRC he always makes some comment depending on who you are, but in this case I got nothing.
No comment, no drop, quest uncompleted.

Same issue on two different characters back to back. Seems to have started in the last few days.

Quest worked fine with my mage (something like 10 days ago) but are bugged for my amazon, have tried it twice and quest item does not drop.
